The RN-Case Manager is a key member of the interdisciplinary healthcare team responsible for facilitating healthcare service delivery. This role integrates Utilization Management, Case Management, and Discharge Planning to ensure optimal clinical and financial outcomes. The Case Manager collaborates with healthcare professionals and resources to develop discharge plans that adhere to regulations and compliance.
Description:
An active member of the interdisciplinary healthcare team the Case Manager facilitates internal and external delivery of healthcare services. The Case Manager integrates the roles of Utilization Management, Case Management and Discharge Planning. The Case Manager is responsible for care coordination and development of the discharge plan through collaboration with the healthcare team and internal/external resources. The job objective is to promote a collaborative interdisciplinary care delivery model to achieve optimal clinical and financial outcomes with adherence to regulations and compliance. Qualifications:


Associate's degree x Required
Bachelor's degree x Preferred






Required Skills

- Must be able to work collaboratively with internal and external resources for optimal discharge planning






Required Licenses
Registered Nurse
- Current/active North Carolina or compact RN license
- BLS optional
